  • Details of your vessel: make, model, year, and registration
  • Your vessel’s current location or last known location

Incident Details:

  • Date, time, and location of the incident
  • A clear description of what happened
  • Photos or videos of any damage, where it is safe to obtain them
  • Police report number in the event of theft or vandalism
  • Contact details of any witnesses or third parties involved

Supporting Documentation:

  • Any police or maritime authority reports, if applicable
  • Quotes or estimates for repair, if already obtained
